Name Change Process

  1. Contact Human Resources to update paperwork, hrstaff@unit5.org.
  2. I-9 Update: Provide proof of name change by providing a Photo ID and a Social Security card with the new name using one of the options below:
    1. Bring in proof of name change paperwork to the District Office OR
    2. Scan & send proof of name change to hrstaff@unit5.org
