Name Change Process
- Contact Human Resources to update paperwork, hrstaff@unit5.org.
- I-9 Update: Provide proof of name change by providing a Photo ID and a Social Security card with the new name using one of the options below:
- Bring in proof of name change paperwork to the District Office OR
- Scan & send proof of name change to hrstaff@unit5.org
- ELIS Licensure Update, if applicable: https://www.isbe.net/
