Address Change for Current Unit 5 Students

If your family has moved to a new address within Unit 5, please contact your child’s school to start the address change process. You will need to:

  1. Gather the three proof of residency documents required to verify your new address.
  2. Complete the Verification of Residency and Enrollment form.
  3. Provide the form and the proof of residency to your child’s school.


Additional information:

  • We cannot update your address without both the Verification of Residency and Enrollment form and the three proof of residency documents.
  • We will update your child’s transportation once we have updated your address. 
  • Your new address may change your school boundary. You can check attendance boundaries on our Boundary Maps. If your new address is outside your child’s current school boundary, your child may stay at their current school for the remainder of the school year if you provide transportation.
  • If the move changes custodial care, legal documentation is required.
