REGISTRATION FOR 2019-2020
Online registration begins at 8 am, July 9Parents of high school students should register online for the 2019-2020 school year. After completing the online registration, students will still need to come to the high school on the assigned day of registration in order to get their books, ID, yearbook picture, and other purchased supplies. They can do this during walk-in registration.Walk-in registration dates are:Early Childhood Schools (Returning students register online. High school students will receive their laptop during walk-in registration. Junior high students will receive their device during the first week of school.Kindergarten Entrance Age RequirementsChildren reaching the age of five (5) on or before Sept. 1, 2019, will be eligible for enrollment in kindergarten. Half day or Full Day Kindergarten The Unit 5 Kindergarten program is a full-day instruction program. Parents who believe their child is not ready for a full-day experience may elect to send their child for half a day. Birth Certificates A CERTIFIED birth certificate must be presented on registration day or the first day of school. The souvenir hospital certificate with the footprints will not be accepted. Immunizations and Health Exams Students entering kindergarten MUST turn in a physical exam and all immunizations by the first day of school or they will not be allowed to attend classes. Parents are encouraged to turn in this information at registration.Other Registration InformationEntrance Age Requirements Children reaching the age of five (5) on or before Sept. 1 2019, will be eligible for enrollment in kindergarten. Children reaching the age of six (6) on or before September 1 will be eligible for enrollment in first grade.Immunizations, Health, and Dental ExamsA student’s parent(s)/guardian(s) shall present proof that the student received a health examination and the immunizations against, and screenings for, preventable communicable diseases, as required by the Illinois Department of Health, within one year prior to:1) entering kindergarten or first grade;2) entering sixth and ninth grades; and 12th;3) enrolling in an Illinois school, regardless of the student’s grade including nursery school, special education, Headstart programs operated by elementary or secondary schools, and students transferring into Illinois from out of state or out of country).All Illinois children in kindergarten, second, and sixth grades are required to have an oral health examination by a licensed dentist and to submit proof prior to May 15 of the school year. School dental examinations must be completed within 18 months of the deadline.Birth CertificatesA certified birth certificate must be presented on registration day or the first day of school for:1) all kindergarten children2) all students new to Unit 5.Proof of ResidenceProof of residence, as required by Unit 5 Board Policy 7.60, must be presented for students enrolling in Unit 5 for the first time or who have left the district and are returning to Unit 5. It is also required for students who have moved over the summer. For additional information regarding registration or opening of school, parents are encouraged to contact the school their child will attend or the Unit 5 District Offices.
