Bias Reporting


Unit 5 has made an ongoing commitment to providing an environment that respects diversity among all members of our learning community. One of the many tools that are being put in place to identify opportunities and areas of concern is the Bias Reporting Form.  This form is to be used to report any and all allegations of bias, racism, discrimination, harassment or any other related conduct not in keeping with Unit 5 values.  

Please note, this form will not capture the email address of the reporter. You will have the option to self-identify within the form and if you choose to do so, you will be contacted by Human Resources to arrange a time to discuss your submission, if desired. The recipients of this submission will be limited to Human Resources, select members of the Executive Cabinet, and any other individuals essential to investigating and resolving the concern(s).  Sensitive information will be removed prior to sharing as needed.
