  • NEW IMMUNIZATION REQUIREMENTS FOR ILLINOIS!

    Beginning this fall with the 2014 – 2015 school year, Illinois law requires ALL students entering school at ALL grade levels (K – 12) to show proof of receiving:
    • 2 (full) MMR  vaccines
    • 1 varicella vaccine
    Children entering kindergarten, 6th or 9th grades must show proof of receiving:
    • 2 varicella (chicken pox) vaccines
    (Most students have already received these vaccines, please check your child’s record.)

    The Hepatitis B series of 3 vaccine requirement moved from 5th grade to 6th grade (most children receive the Hep B series as an infant).

    The Tdap requirement also continues requiring  ALL students entering 6th, 7th, 8th, 9th, 10th, 11th and 12th grades to receive:
    • 1 Tdap vaccine

    Beginning with the 2015-2016 school year students entering 6th and 12th grades (only) will be required to receive:
    • 1 dose of meningococcal vaccine (6th grade)
    • 2 doses of meningococcal vaccine (12th grade)

    UNIT 5 STUDENTS WILL NOT BE ALLOWED TO START SCHOOL WITHOUT THE REQUIRED IMMUNIZATIONS!

    The physical exam requirement is the same – a new exam dated within the past year is required for:
    • Early learner programs (first entry)
    • Kindergarten
    • 6th grade
    • 9th grade

    Make an appointment NOW for a physical and immunizations!  Avoid the rush.  No appointment cards will be accepted in place of a completed physical exam form and immunization record.

    The McLean County Health Department is now accepting private health insurance for adult and child immunizations as well as Medicaid.  Call today for an appointment:  888-5455.

  • PLEASE SUPPORT OUR SCHOOL AND BECOME A SCHWAN'S CUSTOMER
     
    Parkside Elementary PTO has partnered with Schwan's Home Delivery for a fundraising campaign that allows you as a customer to give 20% of your online Schwan's food order back to your child's school!  This 20% return Campaign begins November 4, 2013 and goes until December 19, 2013.  However, if you remain a Schwan's customer and continue to order online, 5% of your food order will be returned to Parkside Elementary throughout the remainder of our 2013-2014 school year.
     
    If you are a current Schwan's customer, all you have to do is place your order online at www.Schwans-Cares.com by using our Campaign ID# 3597 and 20% of your order will be contributed to Parkside Elementary. Your order will be delivered directly to your door on your usual delivery day by your current Schwan's delivery person.  If you are NOT currently a Schwan's customer, but would like to support Parkside Elementary and become one, you can sign up at Schwans-Cares.com by using our Campaign ID.  You will place your food order and enter your name, address, phone number and email, and you will then be contacted by Schwan's regarding delivery.
     
    Since your Schwan's food order must be placed online to qualify, please share our Campaign ID # with any friends or family in any city or state with Schwan's Delivery service, and 20% (or 5% after December 19th) of their order will also be returned to Parkside Elementary!!
     
    Please watch for more information coming home in your child's backpack by the end of this week!!
