NCAA Eligibility Forms

Seniors who plan on playing a sport at Division I or II colleges or universities need to register with the NCAA for Initial-Eligibility. For more information visit the NCAA Clearinghouse. In conjunction with registration...see below.

In order for your "Official Transcript" to be sent to the NCAA, you MUST print the "transcript authorization" forms for both the preliminary & final transcript (2 forms) found on the website. Sign them and take both to Coach Hawkins so he can assist with completing the registration.  This is necessary to finish the registration. Your transcript CANNOT be sent without this authorization.

Juniors & Seniors REMEMBER: Your ACT/SAT scores are NOT on the "Official Transcript". You need to send the scores to the NCAA through the testing companies. When you take the tests use the code 9999 as one of the choices to send the test scores; or you can send an "Additional Score Report" (additional cost) after the test (if you are undecided at the time of the testing).

Attention High School Graduates

The NCAA has adopted new legislation that will require prospects who intend to enroll at NCAA Division I and Division II institutions to supply ACT or SAT scores to the Clearinghouse directly from the testing agencies. Test scores on an official high school transcript will no longer be usable for NCAA purposes. Read more on this under "News and Views" behind the "General Information" heading at www.ncaaclearinghouse.net
