• Screenings will be in the computer lab Wednesday, Oct. 22.

• We will begin screening as close to 8:15 as possible.

• It is impossible to provide an exact schedule of when classes will be called.  We plan to call KG and AM ECE first, then 2^nd, 1^st, 3^rd, 4^th and 5^th.    Only a few 5^th graders will be screened.  They and BD students will be called when there is time between the larger groups of students. PM ECE screenings will start at 1PM.

• Classes need to come for screening when the volunteer helper arrives.   The only exception is if a class is leaving for PE.   The vision and hearing screenings are  mandated by the state.  Other Unit 5 school nurses have been reassigned and the district is contracting with MCHD technicians for this one day to do the mass screenings. It is important that the nurses have a steady supply of students to screen if we are to get through all of the screenings in one day.  We apologize for the inconvenience we know this causes and sincerely appreciate everyone’s flexibility and cooperation.

• Special scheduling requests need to be e-mailed to Mrs. Baue ASAP.  If we are not able to honor your request, we may need to schedule your class on make-up day 11/05.

• Please plan to accompany your class to the screening area and be prepared to escort your students back to their classroom after the screening.  Some teachers read to their students while they are waiting in the hall.

• Please group students wearing glasses at the beginning of the line.

• Not all students will receive both V & H screenings.

Students receiving special services and students new to district will receive both vision and hearing screenings.  Teachers may refer a student for vision and/or hearing screenings. All other students will be screened according to the following guidelines:

KG – V & H

1^st – H

2^nd-  V & H     Even students wearing glasses will be screened for color.

3^rd – H

4^th – V

5^th – Special services, new to district, teacher referrals only – V & H

BD – V & H

Rescreening are scheduled for Nov. 5.  Results of screenings will be available Nov. 5.
