Text Box: Text Box: All medication, including cough drops and non-prescription pain relievers such as tylenol and ibuprofen, must be stored and taken in the nurse’s office. A Request for Supervised Self-Administration of Medication form must be completed by both the prescribing medical provider (physician, dentist, nurse practitioner, etc.) and the student’s parent or guardian. A completed form must be on file in the nurse’s office before any medication can be self-administered by the student at school. Text Box: An exception is made for students with inhalers. Students may keep their inhalers with them or in a convenient place such as their PE lockers as long as a completed medication form has been turned in to the nurse’s office. Parents/guardians may come to school to administer medication to their student, and must do so in the event that the necessary form is not on file at school.
