Text Box: Text Box: All medication, including  cough drops and non-prescription pain relievers such as tylenol and ibuprofen, must be stored and taken in the nurse’s office.  A Request for Supervised Self-Administration of Medication form must be completed by both the prescribing medical provider (physician, dentist, nurse practitioner, etc.) and the student’s parent or guardian.  A completed form must be on file in the nurse’s office before any medication can be self-administered by the student at school.

Text Box: An exception is made for students with  inhalers.  Students may keep their inhalers with them or in a convenient place such as their PE lockers as long as a completed medication form has been turned in to the nurse’s office.
Parents/guardians may come to school to administer medication to their student, and must do so in the event that the necessary form is not on file at school.
Medication must be sent to school in its original  container.
