EXPLANATION OF
REGISTRATION FORMS:
Registration
Information Form
This form
gives general information on a new student including family, address,
and previous school information. This form must be completed in order
to begin the registration process.
Race
and Ethnicity Data Form
This form is used to compile diversity data for the U.S. Department of
Education and is required for all students.
This
letter further explains this new requirement.
Authorization for
Release of Information Form
This form
gives permission to Chiddix Junior High School to request records
information such as grades and test scores from the student’s previous
school. This form can be completed by parents any time during the
registration process.
Child Health Form
This form from the Illinois Department of Health must be completed prior
to the first day of school of an incoming sixth grader or a student
newly registering in the state of Illinois, regardless of grade.
Home Language
Survey
This form
identifies students with needs for bilingual and English as a Second
Language (ESL) services. A copy of this form must be in every new
student’s permanent file and can be completed during the registration
process.
Acceptable Use Policy Form
This form
explains appropriate behavior with regards to electronic information
resources such as internet, email and personal computers. A copy of
this form, signed by both student and parent, must be in every new
student’s permanent file and can be completed during the registration
process.
On-line
Report Card Letter
This form is used to request a hard copy of the report cards and
progress reports for the 2010-2011 school year. Unless this form is
completed with the appropriate boxes marked, grades will only be
available on-line through Skyward. To request receipt of a hard copy of
grades, this form can be completed and turned into the Guidance office
anytime throughout the year.
IL Student Transfer
Form
This form
should be completed by the school the student is transferring from.
Chiddix will require this form on any new student if that student is
transferring from an Illinois public school. Out of state and/or
private schools are not required to provide this information.
