NEW STUDENT REGISTRATION INFORMATION:

Welcome to Chiddix Junior High School!  In order for your child to enroll, the following information is necessary:

1.       Proof of residency – In order to register students new to Unit 5 and Chiddix Junior High School, families are required to submit proof of residency prior to registration.  Examples of acceptable documents are:                         Rental lease agreement

 Homeowner’s contract

 Mortgage statement

 Homeowner’s insurance policy

 McLean County tax bill for your address

2.       Copy of Certified Birth Certificate – Hospital Certificate is not acceptable.  Illinois state law requires a certified copy of your child’s birth certificate on file. 

3.       Proof of legal guardianship of student – If student is not living with both natural parents, proof of the legal custodial arrangement is necessary (divorce decree).

4.       Student in Good Standing Transfer Form - If student is transferring from an Illinois public school, this must be obtained and signed from the previous school.  We must have this form before your child can attend school.

5.       Out of State Students must have a physical examination within one year prior to the student’s start date.  This must be provided to the school within 30 days of the start date.

6.       Records – achievement tests, previous grades, and/or information on any special programming would be helpful.
