Athletic Participation

Each athlete must have the following forms turned into the main office prior to participation or tryouts. These forms will only have to be completed once a year.

• Current Sports Physical (Physicals are good for one year from the date of the physical)

• Copy of the Birth Certificate must be on file

• Athletic Permission/Record Form (Attached)
   

Fees: $75.00 for the first activity and an additional $50.00 for each additional activity. Fees are to be paid after the first team meeting. Do not pay athletic fees at this time.
 

Coaches will not be collecting physical forms, birth certificates or athletic permission/record forms. When a student has a desire to try out for a sport, he/she should report to the main office to receive an athletic clearance slip. The gold slip, which indicates all forms have been properly submitted to the main office, must be handed to the coach before the athlete may participate or try out for that sport.
